For good and valuable consideration, I confirm that:
1. I hereby grant permission to Wagestream and its affiliates to use my testimonial, including my name, image, video recording and likeness, on Wagestream’s website, social media pages, and any other medium in perpetuity. I understand that my testimonial may be edited for length and clarity, and waive the right to inspect or approve the finished product.
2. I understand that my testimonial will be used for marketing purposes and acknowledge that I am not entitled to any compensation for its use. I hereby waive any and all rights to the work product, royalties or other compensation arising from or related to the use of my testimonial by Wagestream.
3. I confirm that the testimonial is my own genuine opinion and accurately reflects my experience with Wagestream. I have no objections to the use of my testimonial by Wagestream.
4. I agree to indemnify and hold Wagestream and its affiliates, officers, agents, and employees harmless from any and all claims, demands, and damages arising out of or in connection with the use of my testimonial.
